Official Terms & Conditions: AUTHORIZED SCHOOL DISTRICT OPERATORS OF K – 12 SCHOOL CAFETERIAS: Beginning on 8/1/09 through 6/30/10 (“Enrollment Period”), complete the enrollment form and mail it into the address indicate on the form or go to www.fafh.com/segments/school/k12connection and complete the online Official Enrollment Form to enroll in the Fall semester (8/1/09 – 12/31/09) (“Promotion Periods”) and/or the Spring semester promotions (1/1/10 – 6/30/10) (Promotion Periods”) to earn tools as noted below. Enrollment in the Fall semester will earn you an automatic enrollment in the Spring semester; or you may enroll in the only the Spring semester by following the instructions on the enrollment form. 1. Feature Kellogg’s® Food Away From Home products from the Power list of products and from the NEW list of products (as indicated below) on your menu during a one (1) calendar month time period during one or both of the Promotion Periods (award cannot be earned by using cumulative purchases throughout each or both Promotion Periods) (see Award Levels - # 3 below). Menu Qualifying Levels: Rewards 1.0 Level: menu Kellogg’s® Food Away From Home products consisting of at least four (4) Power list products and one (1) NEW list product at Breakfast, Lunch, A La Carte and/or After School; Rewards 2.0 Level: menu Kellogg’s® Food Away From Home products consisting of at least six (6) Power list products and three (3) NEW list products at Breakfast, Lunch, A La Carte and/or After School;- Rewards 3.0 Level: menu Kellogg’s® Food Away From Home products consisting of at least eight (8) Power list products and five (5) NEW list products at Breakfast, Lunch, A La Carte and/or After School. For After School Snacks and/or A La Carte Selection Participation: Every ten (10) cases of Kellogg’s® Food Away From Home products (must be same product) from the Power list of products and/or from the NEW list of products equals one (1) menu placement. Purchases must all be made within a one (1) month time period (and the same period as any menu placements if combining with Breakfast and/or Lunch) to qualify. Kellogg’s® Morning Jump-Starts™ counts as two (2) new menu placements. Any Kellogg’s® Food Away From Home product launched after 1/1/2009 qualifies at a NEW product. NOTE: Schools with enrollment larger than 30,000 students can earn double quantities of tools – include enrollment and participation level of your school district on the redemption form. 2. Select the tools for your qualifying level as follows (tools may be mixed from multiple categories):- Rewards 1.0 Level Award: One (1) tool. - Rewards 2.0 Level Award: Two (2) tools.- Rewards 3.0 Level Award: Three (3) tools. Award tools are available while supplies last. 3. Complete the Fall and/or Spring Redemption Form located in the pocket of the promotion overview piece within the Brochure, as indicated, and send it in using an envelope with sufficient postage affixed along with the relevant menu(s) (if placements are branded); or the relevant menu(s) and a copy of all relevant distributor velocity reports (if placements are unbranded); or relevant snacks distributor invoice(s) (if participation is via After School and/or A La Carte selections) to: Kellogg’s™ 2009 – 2010 K – 12 HUB Program, P.O. Box 1007, Brownsdale, MN 55918-1007 to be postmarked by 1/31/10 (for Fall semester), and postmarked by 7/31/10 (for Spring semester).Alternatively, you may complete the Redemption Form online by going to www.fafh.com/segments/school/k12connection, click on “redemption links” and then follow the instructions to upload the required documents. Online submissions must be completed by 11:59:59 PM (ET) on 1/31/10 (for Fall semester) and by 11:59:59 PM (ET) on 7/31/10 (for Spring semester). Limit one (1) tool award redemption per semester – max. two (2) tool award redemptions per district for the Promotion Period (ARV $250 ea. tool award). For A La Carte/After School: snacks distributor invoices must be submitted to show a minimum of ten (10) cases per product; participants may not count the same purchase(s) of Kellogg’s® Food Away From Home products towards both Menu participation and A La Carte/After School Participation. 4. Receive tools award(s) based on your qualifying level of participation. Awards will be delivered to participants upon receipt and verification of qualifying level of participation information. Please allow 4 – 6 weeks from receipt of submission for delivery of award. Offer good only on Participating Products as stated herein. Availability of some product(s) may vary by location. Offer open only to Authorized School District Operators of K – 12 School Cafeteria purchasing from foodservice distributors and located in the fifty (50) United States and the District of Columbia whose account is in good standing with the Sponsor during the Promotion Period and at the time of receiving any award. Sponsor is not responsible for lost, late, mutilated or illegible submissions nor for electronic transmission errors resulting in omission, interruption, deletion, defect, delay in operations or transmission, theft or destruction or unauthorized access to or alterations of entry materials, or for technical, network, telephone equipment, electronic, computer, hardware or software malfunctions or limitations of any kind, or inaccurate transmissions of or failure to receive entry information by Sponsor or presenter on account of technical problems or traffic congestion on the Internet or at any website or any combination thereof. If for any reason the Internet portion of the program is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Sponsor which corrupt or affect the administration, security, fairness, integrity, or proper conduct of this Promotion, the Sponsor reserves the right at its sole discretion, to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify or suspend the Promotion.
